Lip filler is one of the most requested injectable treatments in medical esthetics — and also one of the most misunderstood. If your mental image is "overfilled" or "duck lips," you're picturing the result of too much product, poorly placed, often in the wrong areas. Done well, by a skilled injector who understands facial anatomy and proportion, lip filler is subtle, natural, and deeply satisfying. Here is everything you need to know before your first appointment.
What Lip Filler Actually Does
Lip filler — typically a hyaluronic acid-based product — adds volume, improves definition, enhances shape, and can correct asymmetry. Depending on where and how it is placed, it can make lips appear fuller overall, lift a flat upper lip, define the Cupid's bow, smooth vertical lip lines, or turn up the corners of the mouth. The technique matters as much as the product.
How Much Filler Do I Need?
For first-time clients, less is always more. Most injectors recommend starting conservatively — often 0.5 mL to 1 mL — to allow you to see how your lips respond to the product, assess the results you like, and add more at a follow-up if desired. Lips that are overfilled are much harder to correct than lips that could use a touch more. A skilled injector will always discuss volume expectations at your consultation.
Before Your Appointment
- Avoid blood thinners (aspirin, ibuprofen, fish oil, vitamin E) for 5–7 days before to minimize bruising
- Avoid alcohol for 24–48 hours before
- Stay well-hydrated in the days leading up to your appointment
- Inform your injector of any cold sore history — antiviral prophylaxis may be recommended
- Arrive with clean skin, free of makeup
- Avoid lip waxing or laser in the 2 weeks before
During the Appointment
Topical numbing cream is applied for 20–30 minutes before injection. Most quality fillers also contain lidocaine (a local anaesthetic) within the product itself, which further reduces discomfort during injection. Most clients describe the sensation as mild pressure and pinching. The actual injection process typically takes 15–30 minutes.
After Your Appointment: What to Expect
Swelling is normal and expected — your lips will look larger than their final result for the first 3–5 days. Bruising is possible and resolves within 1–2 weeks. The final result is not fully visible until approximately 2 weeks post-treatment, once all swelling has resolved. This is also when a follow-up appointment can be scheduled if you want to assess and potentially add volume.
- Avoid pressure on lips for 24–48 hours (no kissing, straws, or pressing)
- Avoid extreme heat (saunas, hot yoga, sunbathing) for 48 hours
- Sleep elevated the first night if possible to reduce swelling
- No dental work for 2 weeks post-treatment
- Arnica gel or supplements can help minimize bruising
How Long Does Lip Filler Last?
Lip filler typically lasts 9–12 months, though individual results vary based on the product used, your metabolism, and the amount placed. Many clients choose to top up every 6–9 months to maintain their preferred look. Over time, regular filler can also stimulate your own collagen production, improving lip quality even after the product has metabolized.
