Skin Diva Regina (“Skin Diva,” “we,” “us,” or “our”) is responsible for personal information under its control. We follow applicable Canadian privacy requirements, including the Personal Information Protection and Electronic Documents Act (“PIPEDA”) where it applies. Depending on the information and services involved, Saskatchewan’s The Health Information Protection Act (“HIPA”) may also apply to personal health information.
01
Scope and accountability
This policy applies to information we collect through this website, our online contact and consultation forms, and communications that follow from those requests. It also describes what happens when you use third-party features available through our website.
Skin Diva Regina is accountable for personal information under its control. Questions, access requests, corrections, and privacy complaints may be directed to our privacy contact using the details at the end of this policy.
This website may link to or display services operated by other organizations. Their privacy policies apply when they collect information directly from you.
02
Information we collect
We collect information in the following ways:
- Contact requests: first and last name, email address, subject, and the message you choose to submit.
- Consultation requests: first and last name, email address, phone number, the service you are interested in, and optional treatment goals or notes.
- Communications: information you provide when corresponding with us by email, telephone, text message, social media, or in person.
- Technical information: our hosting and standard server infrastructure may process an IP address, browser or device type, referring page, requested page, date and time, and diagnostic or security logs.
Please avoid including unnecessary medical or other sensitive details in a general website message. Information about treatment goals may constitute personal health information depending on what you choose to provide.
The Skin Quiz processes your answers in your browser to display a result. Quiz answers are not submitted to or stored by Skin Diva unless you separately provide information through a consultation or contact form.
03
How we use personal information
We use personal information for purposes that include:
- responding to questions and consultation requests;
- contacting you about appointments, services, and requests you initiated;
- assessing service interests and preparing for a consultation;
- operating, maintaining, securing, and troubleshooting our website;
- meeting legal, regulatory, insurance, and professional obligations;
- investigating suspected misuse, fraud, or security incidents; and
- using information for another purpose with your consent or as permitted by law.
We limit collection, use, and disclosure to what a reasonable person would consider appropriate in the circumstances and to the purposes identified at or before collection.
04
Consent and communications
We seek meaningful consent appropriate to the sensitivity of the information and the purpose for which it is collected. Consent may be express or implied where permitted by law. You may withdraw consent, subject to reasonable notice and legal or contractual restrictions, by contacting us.
When you submit a consultation request, you authorize us to contact you about that request, an appointment, and related services using the contact information you provide. Transactional or service communications are different from promotional messages.
We send commercial electronic messages only with consent or another lawful basis under Canada’s Anti-Spam Legislation (“CASL”). You may unsubscribe from promotional email or text communications using the instructions in the message or by emailing us. Withdrawing marketing consent will not stop necessary appointment or service messages.
07
Retention and safeguards
We retain personal information only for as long as reasonably necessary to fulfill the purpose for which it was collected and to meet legal, regulatory, professional, insurance, accounting, or dispute-resolution requirements. Retention depends on the type and sensitivity of information, why it was collected, and applicable obligations. When information is no longer required, we take reasonable steps to securely delete, destroy, or anonymize it, unless continued retention is required by law.
We use reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information. These may include limiting access to people who require it for their work, service provider protections, input validation, and operational security practices. No website, electronic transmission, or storage system is completely secure.
If a privacy breach occurs, we will investigate, contain, document, assess, notify affected individuals, and report to regulators where required by applicable law.
08
Your privacy rights
Subject to applicable law, you may ask to access personal information we hold about you, request a correction, ask about how it has been used or disclosed, withdraw consent, or request deletion where we are not required to retain it.
Send your request to jodie@skindivaregina.com. Please describe your request clearly. We may need to verify your identity before responding and may withhold information where permitted or required by law. If we cannot fulfill a request, we will explain why, subject to legal restrictions.
If you have a concern, please contact us first so we can try to resolve it. You may also have the right to complain to the Office of the Privacy Commissioner of Canada or, for matters within its jurisdiction, the Saskatchewan Information and Privacy Commissioner.
