Gummy Smile? Here's How Botox Can Fix It Without Surgery

If more gum than you'd like shows when you smile, surgery used to be the only option. A few precise Botox units can now produce the same result in minutes.

A gummy smile — technically called "excessive gingival display" — occurs when a significant amount of gum tissue shows above the upper teeth during a smile. Most people show 1–2mm of gum when they smile fully; a display of 3mm or more is generally considered a gummy smile. For people who are self-conscious about this, it can significantly affect confidence and even cause people to smile with restraint or cover their mouth when laughing.

What Causes a Gummy Smile?

Gummy smiles have several possible causes: the upper jaw bone may be slightly longer than average, the gum tissue may have overgrown to cover more of the tooth surface than ideal, the teeth may be smaller or shorter than average, or — most commonly — the levator labii superioris muscle (the muscle that elevates the upper lip when you smile) is overactive, pulling the lip much higher than necessary to expose the teeth.

This last cause — a hyperactive lip elevator muscle — is precisely the type of gummy smile that responds beautifully to Botox treatment. And it is by far the most common type.

How Does Botox Fix a Gummy Smile?

When a small amount of neuromodulator is injected into the levator labii superioris alaeque nasi muscle — the primary lip elevator — it partially relaxes the muscle's ability to contract. The upper lip can still lift when you smile, but it lifts a controlled, appropriate amount rather than the exaggerated elevation that was revealing too much gum tissue. The result is a smile that shows your teeth beautifully without the visible gum line.

The treatment itself is quick — typically 2–4 precise injections, 15 minutes total. Discomfort is minimal. Results begin appearing within 3–5 days and are fully visible at 2 weeks.

Is It Right for Every Gummy Smile?

Botox is specifically effective when the gummy smile is caused by an overactive lip elevator muscle. If the gummy smile is primarily structural — excess jaw bone or significant gum overgrowth — orthodontic or surgical approaches may be more appropriate. How Long Does It Last?

Gummy smile Botox results typically last 3–4 months, similar to other neuromodulator treatments. With regular maintenance appointments, many clients find that the results last slightly longer over time as the overactive muscle becomes conditioned to a reduced range of motion.
